I'm a naturally introverted person and I've always had bad stage fright (shaking voice when speaking, trouble breathing, can't concentrate on what I'm trying to say because i'm so aware of the audience watching me, wierd sort of "tunnel vision" like an altered state of consciousness due to my nerves, etc.). However, I'm now a tour guide and I stand in front of up to 16 people and speak to them non-stop for an hour or more. How did I make the change? I got a job where I HAD to do it or be fired. Also, I give 4-6 of these tours a day, so by now it doesn't even phase me.
My advise: Take a public speaking class or anything where you are forced into the limelight. I know this is the last thing you want to do, but DOING IT is the key to CONQUERING it.
Also, when you are speaking to a crowd, focus on one or two people and essentially speak to them. This way you are really only speaking to one person at a time while (it just so happens) that other people can hear what you are saying. Over time as you get better you will speak to one person and then move around from person to person throughout the crowd as you speak. DO NOT (in my experience at least) simply try to ignore the audience and speak out into "empty space" - it makes it impossible because you are trying to communicate with these people and speaking into space always disorients me mentally.
The Basics: Speak or read SLOW. The more nervous you are, the slower you should go and the slower you should breath. There is no such thing as "too slow" if you are nervous.
