i have the cardboard case for the cd album (ultravisitor)
promo, not for the single though... wich i'm pretty sure is
in a jewel case....

if you still want the scan for the cardboard though i can
kindly ask tolstoyed to post it for me.

i guess it's a no then... i checked to see what case the
single promo cd is in and can't find anything though i'm
sure it's jewel...
came across this though wich seems cool.

the album promo on cd is the same cover but in a thin
cardboard cover. all pretty boring. why don't you just
scan the 12" cover as it's the same i think as for the
promo.
