Tetris: not underrated at all.
Lolo 1,2,3: I don't know what this is, but it sounds like you have to complete it by typing "lol" as badly as you can then counting to 3.
It sounds pretty bad.
What happened 5 decades ago?

zelda 2
everyone seemed to hate it at the time, because it was so different from the original.. but i think it has some great innovations which aren't really seen in the other zelda games (the rpg-style leveling system, for one).. if you've never played this one, go download the rom.

Llamatron - although it's got a massive cult following, so I don't know if you'd really consider it underrated.

Yes, i remember Shadowrun for the SNES, and it was decent, but i prefer the Genesis version, as it allows the player much more freedom and deeper character development.
Anyways, a few of mine:
Phantasy Star III - Everyone seems to hate on this game, but it's actually one of the more unique games of the series. The funny thing is, the beginning of the game starts with a princess getting kidnapped, something which would of infuriated Phantasy Star fans at that time considering the game's traditional atmosphere of futuristic fantasy. However, as the game progresses, you start realizing more and more that the game is Phantasy Star after all, up to the plot twist where the entire game suddenly makes sense. T
hat, and the fact that the game spans three generations. The first character is an Orakian, a man of pure fighting prowess, no techniques. However, once you complete his generation, you can marry the Layan, Maia, or the Orakian, Lena. If you marry Maia, your next character is 1/2 Orakian and 1/2 Layan, a balance of good magic and strong fighting. If you marry Lena, your character is 100% fighting. And each of these two possible characters have their own quests, own appearances and scenarios. They each have two girls they can end up marrying, meaning that your final main hero could be one of four possible characters, each with their own ending. Quite ahead of Genesis' time, compared to other RPGs.
I'll probably post a few others here in a moment.

another one, Bushido Blade. While many fighting games were mostly about combos and special manuevers, this game was more about timing and strategy.

for me, definitely Aidyn Chronicles - The First Mage (on N64)
one of the best games i've ever played. Every review told me it sucked, i couldn't disagree more.
Oh, and ofcourse "the Lost Ride' on Philips CD-i. A virtual roler coaster in full motion video, putting the cd-i to the max. But hey, who knows cd-i.... actually the whole cd-i is underrated as hell.
