Universal Indicator Green and Scharlach Eingang by BW.

It's from either Gundam or Armoured Suit Votoms; a Japanese
anime. Whomever was doing Rephlex's cover art then (I think
Grant was; don't quote me on that) was using a lot of
manga-inspired imagery (remember the old Rephlex website
with the Gundam robot on it?) at the time as it was gaining
popularity.

btw: yeah, i remember the old rephlex web site with the
gundam, it was very nice
